SNOWBOARD and or BIKE EQUIPMENT RENTAL & LIABILITY RELEASE AGREEMENT
I accept for use AS IS the equipment rented from BOARD HOUSE AND BIKE SHOP and accept full financial responsibility for the care of the equipment, while it is in my possession. I will be responsible for the replacement at full value of any equipment rented under my name, that is not returned to the shop in its original condition for any reason. I agree to return all equipment by the agreed date and time. I understand that the sport of skiing/snowboarding and other recreational activities involve inherent and other risks of INJURY and DEATH. I voluntarily agree to expressly assume all risks of injury and death that may result from skiing/snowboarding or other equipment use. I agree to release the shop, BOARD HOUSE AND BIKE SHOP, and it’s employees, owners, affiliates, agents, the manufacturers and distributers of this equipment from all liability from injury, death, property loss and damage which results from the equipment user’s participation in the sports associated with this equipment. I, THE UNDERSIGNED, HAVE READ AND UNDERSTAND THIS EQUIPMENT RENTAL & LIABILITY RELEASE AGREEMENT. BY SUBMITTING THIS FORM ELECTRONICALLY I AGREE TO ALL TERMS AND CONDITIONS HEREIN.
